Voluntary Participation: I understand that participation in the Black Fox youth wrestling camp involves physical activity and carries inherent risks, including, but not limited to, the risk of injury, illness, or other physical harm. I voluntarily choose to allow my child to participate in this camp.
Assumption of Risk: I acknowledge and assume all risks associated with wrestling activities, including physical contact, use of equipment, and the possibility of accidents or injuries occurring during camp activities. I understand that while the camp will take reasonable precautions to ensure safety, injuries may still occur.
Release of Liability: I release and discharge Black Fox, its organizers, staff, volunteers, coaches, and affiliates from any and all liability, claims, demands, or causes of action arising out of or related to any loss, damage, or injury, including death, that may be sustained by the Participant while participating in the camp, whether caused by negligence or otherwise.
Medical Consent: I authorize the camp staff to provide or seek medical care for the Participant in case of an emergency. I accept responsibility for any associated medical costs.
Non-Refundable Payment Policy: I understand and agree that all payments made for participation in the camp are non-refundable, regardless of circumstances, including withdrawal from the camp or inability to attend.
Photo/Video Release: I grant permission for the Participant’s image or likeness to be used in promotional materials, including photographs or videos taken during the camp, without compensation.
Compliance with Rules: The Participant agrees to comply with all camp rules and instructions provided by the staff. I understand that failure to comply may result in the Participant’s removal from the camp without refund.
